Our Quick Check Program
The goal of our nonprofit is to help professional caregivers--nurses, certified nursing assistants, social workers, volunteer coordinators, physicians, and chaplains--provide the best possible care to low-income, Palliative Care and Hospice patients.
Thoughtful, imaginative caregivers understand that during such difficult times the most modest of gifts can dramatically improve a patient's care, mood, and outlook on life. See our "SUGGESTED GIFTS" page for ideas.
Our Quick Check Program is simple: We mail checks of up to $150.00 to caregivers like you to help pay for gifts you have identified as important to your patient's' improved well-being.
LIMITATIONS OF THE PROGRAM
1. The maximum amount for each patient is $150.00.
2. We regret that our assistance DOES NOT INCLUDE : Paying for items or services that are"medical" in nature, OR helping out with rent, mortgage payments, utility bills or funeral expenses.
OUR SIMPLE APPLICATION PROCESS
Step 1. Determine the specific item(s) or service(s) you wish to purchase.
Step 2. Fill out (completely) the application form on our website.
VERY IMPORTANT: Make sure you give us your organization's complete MAILING address INCLUDING any SUITE number!!! -- Otherwise the postal service WILL return our checks as "undeliverable."
Step 3. Send us your completed application by simply clicking the "Submit" button.
TILT will notify you of our decision via email or telephone--It is our goal to respond within a 48-hour period. If your request is approved, a check will be promptly mailed to your workplace in the approved amount.
Once you receive our check, purchase the approved item or service and make it available to your patient in a timely manner.
PLEASE remember to email us a photo-copy of your receipt to prove purchase. This helps us prevent against fraud.
ALSO, we also ask that you notify us when you get our check, so that we know it has been timely received.
