Our Quick Check Program
The goal of our nonprofit is to help you, professional caregivers, whether you are a nurse, certified nursing assistant, social worker, volunteer coordinator, or a chaplain, provide the best possible care to low-income patients suffering from a life-threatening medical condition. Your patients may be in hospice, or sitting in an infusion room being treated for cancer.
Thoughtful, imaginative caregivers understand that during such difficult times the most modest of gifts can dramatically improve a patient's care, mood, and outlook on life. See our "SUGGESTED GIFTS" page for ideas.
Our Quick Check Program is simple: We mail checks of up to $150.00 from our fund to caregivers like you to help pay for gifts you have identified as important to your patients' improved well-being.
LIMITATIONS OF THE PROGRAM:
TILT is funded solely through donations. To stretch our available dollars, and to ensure that there are enough funds to meet the needs of as many patients as possible, please note the following:
1) The maximum distributable amount per granted request has recently been increased to $150.00.
2) In addition, there is a limit of one approved request per month per same patient.
3) And lastly, our assistance precludes paying for items or services that are "medical" in nature or having to do with, but is not limited to, utility payments, food supplements, or funeral expenses.
STEP BY STEP INSTRUCTIONS:
Step 1. Identify the patient's non-medical need.
Step 2. Locate and identify the specific item or service you wish to purchase.
Step 3. Fill out (completely) the request form found on our website.
Step 4. Send the completed form to us by simply pressing the "Submit" button.
Step 5. TILT will notify you of our decision via email or telephone--It is our goal to respond within a 48-hour period.
Step 6. If your request is approved, a check will be promptly mailed to your workplace in the approved amount.
Step 7. Once you receive our check, you must promptly purchase the approved item or service and make it available to your patient.
Step 8. Proof of purchase must be sent to our office within seven (7) business days from the date of purchase. We accept photocopies sent by mail, or as an email attachment.
